    About This Position

    We want to build a core team that will drive key transformation in US Healthcare management services.

    For this position, we are looking for an experienced Nurse or a Certified Case Manager with at least 5 years of clinical experience with direct patient care. An active License is NOT required. The RPM/ CCM Care Program – Rev+ at Neolytix- is rolled out at multiple medical practices nationwide. The Rev+ Care Coordinator position is a vital member of a cross-functional care team driven by their mutual devotion to serving others. Client's Practice Management leans on this team to supplement the care they cannot provide themselves. This individual will have a team of Case Managers who will be working on patients – monitoring and supplementing the Care Coordinator with additional information that maximizes care and reimbursement for the practice.

    Roles & Responsibility

    • Onboard new Practices and operationalize a Care plan for their patients
    • Manage a team of case managers – Delegate & monitor RPM and CCM-related targets assigned to each case manager.
    • Coach and mentor case managers to improve productivity and care being provided to patients.
    • Act as the key point of contact between the Neolytix team and the Practice Provider
    • Identify patients for specific case management and/or chronic condition management activities using established screening criteria.
    • Apply nursing processes to actively manage members and achieve integrated health services through coordinated service efforts.
    • All other duties, as assigned.

    Skills & Experience

    • Licensure: Active license is NOT required.
    • Education: Bachelor of Science in Nursing (BSN) or Masters in Nursing.
    • Certification: Certification in Case Management (CCM) is preferred.
    • Experience: 3+ years of clinical experience with direct patient care required; 5+ years preferred.
    • Experience in case management/coordination of programs and services preferred.
    • Excellent verbal and written communication skills. Comfortable communicating with members and providers via phone regularly throughout the day.
    • Ability to communicate effectively with members and providers in implementing clinical services; Translate complex clinical concepts for non-clinical audiences.
    • Strong administrative/technical skills; Comfort working on a PC using Microsoft Office (Outlook, Word, Excel, PowerPoint), IM/video conferencing (Teams & Zoom), and telephones efficiently.
    • Trustworthy and accountable behavior, capable of viewing and maintaining confidential personal information daily.
