- The Chart Research Specialist is responsible for the review of medical records for the necessary documents for proper indexing.
- Ensure medical records are complete within the electronic medical records system by identifying and obtaining any missing chart information including signatures from physicians and other pertinent personnel.
- Reports on incomplete medical records based on the deadlines given.
- Conducts Review of Systems (ROS), Exam, HPI, and MD Notes.
- Retrieves existing medical records from Electronic Medical Records (EMR) system.
- Scans and input documents.
- Verifies that information is indexed appropriately.
- Pulls down medical records through Automate Tasks.
- Logs into hospital systems and downloads registration logs and demographic data.
- Documents work processes as required.
- Creates new entries and sub files as needed with correct medical record number.
- Monitors the performance of the Automate Tasks.
- Performs special projects and other duties as assigned.
- High School Diploma or GED
- At least one (1) year of experience handling patient health information and/or medical records preferred
- Working knowledge of medical terminology, anatomy and physiology, legal aspects of health information.
- Working knowledge of department roles and responsibilities.
- Knowledge of Electronic Medical Records (EMRs) such as Cerner, Meditech, Epic, PICIS.
